APPLICATION FOR ENROLLMENT SUMMER 2006

Welcome to Boca Prep Summer Camp! This summer is sure to be a memorable one for your child. Your child will always remember the friends made and the exciting experiences at Camp. We offer an array of activities. These are designed to enrich your child’s creativity, talents, individuality and so much more.

AGES
We welcome children between the ages of 3 and 13 years of age. Preschool children must be 3 years old by January 1, 2006 and toilet trained. Our C.I.T. (Camp Counselor) Program is available for children ages 14-16.

SESSIONS
You may enroll in as many sessions as you wish. Each session is two weeks long but you may split the sessions.

                      Session B: June 12th-23rd                               Session D: July 10th-July 21st

UNIFORMS
All campers must wear a SunLake T-shirt to camp EVERY DAY, unless otherwise noted. Please see our Uniform Policy and Order Form for detailed information. Absolutely no denim or boxer shorts of any kind will be acceptable.

PICK-UP AND DROP-OFF POLICIES
Campers not enrolled in our Sunrise program may not be dropped off earlier the 8:45 am. Campers not enrolled in our Sunset program may not be picked up later than 3:45 pm. Families who drop off or pick up their children outside these parameters will be charged a fee.

MEALS
A well-balanced menu is designed each week to include foods which children enjoy; including burgers, pizza, hoagies and peanut butter & jelly sandwiches. Snacks and drinks are also provided. Students are encouraged to bring water bottles each day to camp.

MEDICAL POLICY
Each camper must submit the appropriate medical form, including HRS-H Form 3040 (yellow form) and a Florida Certification Immunization (blue form) from his/her doctor. During camp hours, all medications are kept in the infirmary and dispensed by our licensed nurse. Parents must sign a medication permission form in the event medical attention needs to be administered.

Mini-Camp
All 2-week Mini-Camp sessions = $300.00 per session
Campers in Full Day Program= $75.00 per session for Mini-Camps

Monday – Friday 8:45 am – 3:45 pm
Sunrise Program: 7:45 am – 8:45 am         $75 per session per child (10.00/day)
Sunset Program: 3:45 pm – 5:45 pm         $130 per session per child (15.00/day)
Sunrise/Sunset Program: $180.00 per session (25.00/day)
*If not enrolled in Sunset Program, picking a child up after 3:45 p.m. will result in additional fees.

In the event of any injuries while my child is in camp, either on or off SunLake’s campus, I hereby give permission to any responsible person employed by the camp to pursue necessary emergency treatment.

I agree to hold those people of authority harmless from any action in this respect.

I assume all rights and risks and hazards incidental to my child’s participation in the activities of SunLake Summer Camp and Academy, INC and its owners, officers, and employees.

I also give SunLake and its affiliated companies permission to photograph and/or videotape my child for use in advertising and promotional materials. Yes No

I understand that SunLake Summer Camp and Academy reserves the right to dismiss any child from the Camp and or Academy program, either temporarily or permanently, due to (continuous) inappropriate conduct, or conduct that compromises the safety and/or happiness of others. Campers or students who are dismissed from SunLake at any time during the summer due to such conduct will not receive any refunds.
